Kansas HealthWave provides health insurance to children under the age of 19 in Kansas.

Before you apply

Before you apply for Kansas HealthWave, gather all of the following:

  • Your computer needs to be able to view pdf files.  You can download the latest Adobe reader here if needed: http://get.adobe.com/reader/
  • Copies of proof of your identity for who is applying to be covered.  For example, if you are a parent applying for your children to get coverage but not yourself, you need to provide identification for your children,.  For children younger than 16, provide a copy of a medical or school record that identifies them.   Call 1-800-792-4884 with any questions.      
  • Proof of income for the parent(s) or guardian(s) for the last three months.  Learn more about proof of income here.  For any kind of income, you need to provide a copy of all check stubs for the last three months.  If you are self-employed, provide a complete copy of your most recent tax return.

How do I apply?

1.      Print out an application at: http://www.kansashealthwave.org/documents/EngApp06_000.pdf. 

2.      Fill out all the information.  Call 1-800-792-4884 with any questions.  

3.      Mail the form, your copies of proof of income, and proof of  identity  for those needing coverage to the Topeka HealthWave Office located at:

Does my child qualify?

To qualify, a child must be a resident of the state of Kansas, a citizen of the United States, and the family must meet income guidelines. 

Income guidelines are based on how much your family earns PER MONTH and the number of people in your family (including the child’s parents).  The amount shown here is the highest income your family can make in a month and still  qualify.  

For a family of 2: $2,429

For a family of 3: $3,052

For a family of 4: $3,675

For a family of 5: $4,299

For a family of 6: $4,922

Here is an example: if a mother lives alone with her child and makes $2,429 or less, her child is eligible to be covered with Kansas HealthWave.

If a mother and father are living with two children and the parents make less than $3,675 combined monthly income, their two children qualify for coverage.

Other family members who may be covered
