MO HealthNet provides health insurance to children under the age of 19 and pregnant women.
How do I apply?
There are four ways to apply: over the internet, printing and mailing a form, calling a toll-free number to request an application, or visiting a local Family Support Division office.
Over the Internet

- Go to http://www.dss.mo.gov/mhk/pdf/886-4367_0108.pdf.
- Print out the document and fill out all the information. A Spanish language version is also available.
- Mail the form to the MO HealthNet Service Center located at:

Do I Qualify?
To qualify for MO HealthNet you must meet income guidelines and be a citizen of the United States or a registered alien.
Income guidelines are based on how much your family earns PER MONTH and the number of people in your family (including the child’s parents). The amount shown here is the highest income the family can make in a month to qualify.
For a family of 2: $3,643
For a family of 3: $4,578
For a family of 4: $5,513
For a family of 5: $6,448
Here is an example: if a mother lives alone with her child and makes $3,643 or less, her child is qualified to be covered with MO HealthNet.
If a mother and father are living with two children and the parents make less than $5,513 combined monthly income, their two children qualify for coverage.
Note: Income guidelines change if your child is currently insured. You have to make about half as much to qualify. Call 1-888-275-5908 with specific questions.
